Every participant must have these signed and turned in before Trek. PDFs will be posted here and in our Google Drive as they're finalized.

Note: Your youth cannot attend Trek without these forms signed by both the participant and a parent/guardian. All participants must also sign the Hold Harmless Agreement for the property owner.
